PELVIC INFLAMMATORY DISEASE (PID)
Gynecologic disorders like Pelvic inflammatory disease such as Salpingitis and endometritis. Pelvic inflammatory diseases is a polymicrobial infections of upper genital tract associated with sexually transmitted organisms like chlamydia trachomatis and neisseria gonorrhoeae and various other organisms like haemophilus influenzae, anaerobes, streptococcus, It is common in sexually active women with multiple partner. Pelvic inflammatory disease is a main cause of ectopic pregnancy and infertility.
SYMPTOMS
In PID infection patients may have abdominal pain on lower side. Fever,chills,adnexal and cervical tenderness, menstrual cycle disturbances.
Upper right quadrant pain (Fitz hugh and Curtis syndrome) may indication of perihepatitis.
Some women may asymptomatic because of low back pain,urinary frequency and post coital bleeding.
TREATMENTS
As soon as antibiotics should be start to increase effectiveness against N gonorrhoeae and C trachomatis and other pathogens.
There should be treatment done for both the partner or sexual partner.
Mild to moderate infections treatment is high success rate.
Drugs such as ceftriaxone, cefoxitin, doxycycline and metronidazole may be used.
Other regimen is intravenous dose of clindamycin and gentamicin.
If there is tubo ovarian abscess is present then clindamycin or metronidazole is used with doxycycline for complete 14 days treatment for anaerobic coverage.
Surgical incision is required in unilateral adnexectomy, for unilateral abscesses.
Hysterectomy and bilateral salpingo oophorectomy in case chronic infections and pelvic pains.
